The Centre Declaration Sheet is provided for the centre to:

  • authenticate the candidates' centre-assessed work;
  • confirm the internal standardisation of marking.

Candidate Record Forms for coursework and portfolio units

AQA issues Candidate Record Forms for all coursework and portfolio units. They serve two purposes:

  • for candidates and teachers to authenticate the work;
  • for teachers to record their assessments and supporting information.

Select Candidate Record Forms for:
A-levels (including Applied A-levels)
AQA Baccalaureate (Bacc)
AQA Certificates
Diploma Principal Learning (PL)
Entry Level Certificate (ELC)
FCSE
Free Standing Mathematics Qualifications (FSMQ)
Functional Skills
GCSE and Applied GCSE (outgoing)
GCSE (new)
Level1/Level 2 Certificate and QCF Qualifications
Project Qualifications
Vocationally Related Qualifications (VRQ)

Private Candidates

Private Candidates who are studying through an accredited correspondence college (method 2) or studying independently (method 3) should use the Private Candidate Record Form.

Candidates who are studying at the centre of entry (method 1) should complete the standard, subject specific Candidate Record Form listed above.
